Thought I'd post a small recipe because a lot of threads ask similar things.
Maybe useful as a starting point: MLX on B200, keeping tokens/sec in mind from the start.
If someone wants the exact settings I can clean them up and post them.
For context, I'm not chasing leaderboard numbers. I mostly want the system to feel consistent day to day.

A couple things I'd check first. On mini PC, the easiest win is often to back off the aggressive settings and see whether the problem survives. A lot of these threads end up being one bad config plus a pile of conclusions drawn from it. Turns out 'just one more test' was a lie.

Did a small comparison this afternoon. The issue went away once I changed one setting and restarted the stack cleanly.

What exact setup are you running? Is this local-only or are you routing anything externally?

I'd approach it in a boring order. Start by verifying the driver/runtime combo, then test with a smaller prompt, then look at router policy.

What worked for me was pretty simple. On 4090, the easiest win is often to back off the aggressive settings and see whether the problem survives. A lot of these threads end up being one bad config plus a pile of conclusions drawn from it.
